No idea but here if you do not have private insurance you can not go to hospital , only if you are dying
As my GP TOLD ME .... YOU WILL NOT see specialist doctor ever again here , you do not have private insurance , all you can have is GP to give you prescription ..... in short I did stop going to see a doctor , and it is not just me here

Of course the situation is not the same everywhere (Nothing is ever 100%).
But there are quite a lot of countries that are struggling with too high numbers of infections at the moment, These countries will be even worse off in your so called free-world.
And of course it's also a question of perception of what is acceptable or not during a pandemic, Still!! 200 million as you describe is not exactly a low number.
BTW : THERE ARE NO FREE WORLDS ANYWAY.
